• Create a virtual patient chart for you using our secure & HIPAA-compliant patient portal, called Simple Practice

• Send you our intake forms via a secure portal invite

  • These forms are similar to those you see at a normal doctor's office, including consent for treatment, privacy policy, etc. Please note there is one form, titled "Intake questionnaire," that will take about 10-20 minutes to complete. This "mini-assessment or evaluation" will help jump-start your therapy sessions and help your Doctor determine the best course of treatment. ​
