• Create a virtual patient chart for you using our secure & HIPAA compliant patient portal, called Simple Practice

• Send you our intake forms via a secure portal invite

  • These forms are similar to those you see at a normal doctor's office, including consent for treatment, privacy policy etc. Please note there is one form, titled "Intake questionnaire" that will take about 10 minutes to complete. This "mini-assessment or evaluation" will help jump start your therapy sessions and help your Doctor determine the best course of treatment ​

• Once all intake forms are signed and filled out completely, we will reach out to get you an intake appointment as soon as possible.
